DisplayLink Survey?

I received a survey by email just now, purporting to be from DisplayLink via a third-party marketing firm called Constant Contact.

Normally I'd forward the email to the alleged originating company so they are aware of any phishing going on, but I can't find any contact info for DisplayLink.

If someone from DisplayLink wants a copy, let me know where to forward it to.
