The driver and host software seems to have installed correctly. The iPad sees the device automatically and accepts a basic (no special characters) password. Windows recognizes that a second display is attached. I've tried it in mirror mode in addition to extend.

OK, not sure if it was EBKC but it started working fine after a reboot.
